People with a learning disability often have poorer physical and mental health than other people. This does not need to be the case. Annual health checks are for adults and young people aged 14 or over with a learning disability.
At Appletree Medical Practice we are pleased to offer annual health checks for patients with learning disabilities, to try to minimise the health inequalities that many people with a Learning Disability face. This can be done by identifying conditions such as diabetes or heart disease at an earlier stage or by supporting people to reduce their risk of developing them in the first place.
If you are on the Learning Disability register you will receive an invite from the practice to book an annual health check.
